Palliative and end of life care dashboard development

Digital Health and Care Wales is supporting the national ambition to reduce health inequalities and improve access to high‑quality palliative and end of life care, including specialist services.

Working in close partnership with the Palliative and End of Life Care Programme, DHCW is enabling a data‑driven approach that helps the system better understand patient needs, outcomes and experiences in the last year of life.

Central to this collaboration is the development of a dedicated Palliative and End of Life Care Dashboard, designed to provide the national clinical team with timely, actionable insights. The dashboard supports service planning and redesign by offering a consistent national view of activity, outcomes and variation, helping to promote more equitable and coordinated care across Wales.

Through this joint programme, DHCW has made a significant contribution to the development and delivery of digital tools, data pathways and analytical outputs, including:

  • The creation of a Patient Reported Outcome Measures (PROMs) pathway, including the first carers’ outcome measure developed for this population, enabling richer and more meaningful insight into patient and carer experiences.
  • The integration of PROM visualisations within the Welsh Clinical Portal, supporting clinicians to use patient‑reported outcomes directly within care settings.
  • The development of robust evidence outputs, including two academic manuscripts focusing on the use of PROMs and insights from the last year of life, strengthening the evidence base and informing national service improvement.

DHCW’s dashboards, data services and analytical expertise continue to underpin this work, ensuring clinicians, programme teams and decision‑makers have access to high‑quality, timely intelligence. This supports the delivery of compassionate, well‑coordinated palliative and end of life care for people across Wales.
