The Diabetes Consultation Note has launched in Withybush Hospital in Haverfordwest. The note enables Diabetes consultants, specialist nurses and dietitians to record and view patient information within the Welsh Clinical Portal (WCP).
The note holds the most important details about a diabetes patient, all in one place. Previously diabetes information was recorded and held in separate files, and difficult to bring together. The note can also be shared with a patient’s GP.
It includes readings, observations, diagnoses, medication including complex insulin regimens, blood glucose readings, clinical assessments and clinical notes. It shows them the patient's most recent pathology test results relevant to the management of the condition e.g. HbA1c results. It can be used to support multiple settings in secondary care including outpatient clinics, inpatient stays, telephone contacts and group education.
The note first went live in Prince Charles Hospital in Merthyr Tydfil last year. Diabetes Consultant, Dr Gautam Das, said it was a ‘game changer’ in the way diabetes is managed in Wales. He said, “It brings all health care professionals involved in diabetes care under one roof and ensures smooth, streamlined and continuous care for our patients”.
