We are providing the technology needed to help support the delivery of integrated health and social care in the community and in the patient's home.
Welsh Government wants everyone to have long, healthy lives. Through the Healthier Wales plan it has pledged that health and social care services will use the latest technology and medicines to help people get better, or to live the best life possible if they aren’t able to get better.
More services will be provided outside of hospitals, closer to home, or at home, and people will only go into hospital for treatment that cannot be provided safely anywhere else.
We've already begun by introducing the Welsh Community Care System to join up the care provided by health boards and local authorities.
At the community pharmacy, our software is driving the Choose Pharmacy service, which is allowing community pharmacies to provide free NHS Wales services for a range of common ailments, emergency medicines provision and medication reviews - particularly useful for patients recently discharged from hospital. This reduces the pressure on GP practices and gives patients more choice.
We're supporting child health through the Children and Young Persons Integrated System (CYPrIS). This is the fourth version of a system, designed and built by NHS Wales, to meet the needs of the national immunisation programme, data and statutory reporting requirements that assist the NHS and Public Health in Wales.
